What about it is uncomfortable? Have you tried wearing a tucked undershirt? That can help with some of the direct irritation of the skin if that is the issue
Haha. I started carrying AIWB because of this forum... more or less around when I registered in sept 2011.
It applies direct pressure on the scarline, and I also have some nerve damage resulting from a poorly placed drain. I’ve tried undershirts and pads, but they just don’t work for me AIWB. IWB does work at 3:30 if my pants are loose enough, but that makes my jeans unacceptably baggy. I’m much happier with a well designed OWB and a cover garment, Tony’s OWB2 is the best thing I’ve ever found.

Fit of the pants is a huge factor for me. All my AIWB holsters are JMCK which are top notch in design and workmanship, yet I've had mixed results. But not because of the holster. I use a Graith Specialist Belt.
I recently had my Gallbladder removed and after surgery I started wearing relaxed fit jeans 2 inches larger in the waist than before surgery. (Imagine 4 stab wounds in your 6 pack abs... Ok maybe a 12 pack...)
The extra space makes a huge difference for me in both comfort and concealment with AIWB as the pants fabric isn't pushing the muzzle area of the holster in as much and defeating the foam wedge, hence pushing the grip out. I'm actually wearing my Sig 229 and spare mag now AIWB testing it out post surgery. Not sure I can stick with it because of the surgery issue, but looser pants do make a difference at least for me. YMMV

Werd! Pants that ride low on the waist (which is apparently fashionable at the moment) flat do not work with AIWB for me (not to mention it just feels like my britches are falling off. Sadly, even some "work" pants like Dickies and Carharrt have succumbed to this trend. Actually, a ton of brands now have this low-ride "feature". I gots to have man-pants! My most recent faves are the Ballroom Khakis (full cut) from Duluth. I think their slim cut britches are also low ride.

I went to AIWB when IWB became to uncomfortable with my bad hip. It also helped at the time I lost a good chunk of weight. Now with some (thankfully not all) weight back on, some of the AIWB doesn’t work as well. Fortunately during my obsessive days, I acquired over a dozen AIWB holsters, so I do have one that seem to work with the extra fluff.
The other thing I’ve found regarding IWB is it’s much more tolerable now with my Tenicore Zero belt. It turns out you don’t need a Hercules strength belt of steel to IWB, and allowing the holster a slight amount of twist takes some pressure off the hip area.
Occasionally necro-threads pay off.
