You've probably already checked, but I didn't see it mentioned. Is the little e-clip on the other side of the screw still there? Without that it can have issues. If it's missing, I have in in my notes somewhere what the size is and you can source them at Home Depot.

Was the small spring still mated with the screw? Maybe try a bit of thread locker on the screw.

Good call, putting some vc3 on there. I did notice the screw is slightly bent. I’m wondering if that maybe is the issue here. Maybe I’m over torquing?
