From one of the local PD's. Gun was a PX4, .40 S&W.
During firearms training on Wednesday April 25, 2012, an officer was attempting to complete a course of fire when a round in the chamber did not fire. The officer attempted to perform an immediate action drill in order to remove that round and continue firing. While performing this drill, he did not rack the slide back hard enough and then rode the slide forward causing the round to tumble and catch in the chamber.
Realizing that he had a bigger problem and needed to do a remedial action drill, he brought the gun in closer to his body to perform this action; Pushing much harder and faster this time. Because of the improperly performed immediate action drill the round in the magazine had started to enter the chamber at a weird angle and the bullet that had failed to eject was pressing down on that round.
When the slide was pushed back so violently the second time, it caused the primer of the round to strike a sharp edge of the gun or magazine with enough force to discharge the round. The round discharged within the gun while still partially in the magazine and not seated in the barrel causing it to destroy the inside of the weapon and blowing pieces of the frame and trigger bar out the right side of the weapon.
No injuries were sustained due in part to all the officers on the range wearing their eye and ear protection and ballistic vests per range rules and instructions. Additionally the officer was left handed. If he’d been right handed he would have sustained serious injuries due to the location on the gun that exploded.
