Dont give up man. Youve got to continue the fight. Eventually you will find someone who can help you, probably something or someone youve never expected.
From personal experience an old school Yoga teacher and a COMT Physical Therapist would be worth a try.

I second this. I have a tenant who was literally trying to have his seemingly healthy leg amputated after more than a decade of unsuccessful treatments for a bad knee injury. I mentioned it here & UNK told me about COMT PTs so I passed the word along.

Long story short, he saw a COMT ~6 months ago who worked with him a bit, diagnosed some nerve issue, and referred him to a specialist surgeon. After finally getting surgery approved & another ~6 weeks of PT he's now walking without a cane or painkillers.

Wes' Voyager might work for you in the AOWB position.

I was having trouble rotating my shoulder back for 3 or 4 o’clock EDC and JMCK made me a custom version of their OWB with a 10 degree reverse cant for my G26. I wear it forward of my hip and its very comfortable and fast. Works well for me…
    As always, YMMV.
