It looks like he removes the magazine and removes the chambered round slowly.
But there is another “ghost round” in the gun that fires when he fumbles the trigger.
All poor gun handling aside, people were wondering where the extra round came from.
So I did some noodling with snap caps.
So it’s possible for a loose round in the grip to feed in when unchambering.
The thought was a dirty magazine or a sticky follower could allow a round to fall out of the top when tilting down.
Not that anyone here wouldn’t, but there’s a reason to visually or tactically check the chamber.
