Steve, sorry this happened to you.
Would yo be able to expand on the ammo? I’m not clear what “commercial reload ammo“ you were using exactly?

What is the headstamp of the brass that failed? Some brass like AMERC is suspect.
Wolves don't kill the unlucky deer.
I would contact Washougal River CC and tell them what happened. According to this post on AR15.com they are not making the hights quality ammo. I tend to avoid gun show reloads. Buy some factory new ammo and I bet you will have a much better experience.
We could isolate Russia totally from the world and maybe they could apply for membership after 2000 years.
"Commercially reloaded ammo" is the problem, not the pistol.

Beware of my temper, and the dog that I've found...
Thank you and I have learned my lesson, from now on I will buy factory original ammo.
PS headstamp on the case is FC 9MM Luger
I found another post about APX going kaboom and interestingly enough, the casing sits in the barrel lower than mine.

Will find out what Beretta says, hopefully soon.
Thank you all for your help.

Hey @op, just for giggles I dropped a dummy round in one of my four APX pistols, and it looks exactly like yours. Considering this gun has a couple thousand rounds on it, I concur with the assessment of my peers and that the ammo is the culprit.
Life is too short for cheap ammo, my dude
About 15 years ago I was standing next to someone firing a Garand when a gunshow reload let go. Fortunately all I caught was unburnt powder instead of shrapnel. We found the piece of brass and it had failed in the neck/shoulder area. Why it didn't go high order or let go OOB is still a mystery to me.

Totally understand about the reloads now.
Since I am somewhat new to shooting, I didn't even thinking about reload vs factory rounds.
Thank you all for your help.
