How does it run with bulk garbage like Remington thunderbolt?
This made my day. First, a .22 "Box to Match" review. I love rimfire guns and have more than I can reasonably justify. Second, and even cooler, Mr. Kelley is back.

Gun has a bolt running in a receiver with a long "bustle" back over the hand.
What does that do to the balance? Most pistols have a degree of "muzzle preponderance" (Extra credit for the author of that term.) and we are used to and usually like it.
What does that do to the sighting? Sight far to the rear of hand will amplify the visual wobble.
Looks like they took a carbine wannabe burp gun and cut it down into a pistol to get on the market first. Watch for the long version.

The best part of the video is that the unbridled joy of shooting the Kel-Tec CP33 just jumps out of the screen. That reaction may have convinced me to buy one.
It's really great to see Kelley shooting again. Hope he's able to continue doing what he does.
This is one Kel Tec I could see buying. The 22 mag version seemed interesting but I couldn't see plinking with a 22mag.

How big is the grip, and how well would it work for a shooter with small hands?

It's always driven me nuts that I couldn't get a fun plinking 22 pistol that holds more than 10 rounds. KT may have solved that, will definitely be looking into this...
