I'm thinking both students and instructors may be getting fatigued and letting their guards down...

I think that's an excellent observation. We tend to forget that concentration is real work. We understand muscles get tired, we're less understanding of our brain being tired. I'm constantly reminded when I dry fire. Concentration and dedicated practice slips into going through the motions fairly quickly if you don't watch it.

It bothers me somewhat less when it's after unload & show clear. The times it really tweaks me out are when people go too fast on make ready, don't look, and end up pointing the muzzle of the gun right into their own hip joint and/or ass cheek, sometimes repeatedly, before it finds its way into the holster.
