I've been wearing the same two Wilderness Instructor belts since 2007.
One's a five stitch and one is a CSM. Neither hasn't failed me yet and don't see the need to try anything else.

Another vote for the Graith Specialist! I’ve never before experienced their level of customer service and the Specialist is an outstanding belt.

Everyone should have a Wilderness 5 Stitch belt in the closet, though I like the Talon Tactical belt as well. I can recommend either.
If you application is specifically AIWB, I'll add my voice to those recommending the Graith Specialist. I prefer the Wilderness or Talon Tactical designs for OWB as they provide a bit more stiffness to support the weight of the holster, but the Specialist is a favorite for AIWB, thanks to the minimalist buckle and additional flexibility.

I'm a big fan of the Graith Specialist. I have a couple and wear them everyday unless I need to dress up, then nice leather from JRC comes out.
The budget option is an Uncle Mike's Instructor Belt.

Wound up with a Mastermind* AIWB a couple of years ago as a PF Supporter gift.
Absolutely perfect. The current Specialist looks pretty similar, and I'd get one in a heartbeat.
*now Graith
