I would AIWB that with a beehive round.
I was into 10mm Auto before it sold out and went mainstream, but these days I'm here for the revolver and epidemiology information.
I can imagine some dummy chambering a Mk19 round in that and trying it. Once.
I worked for someone that carried something similar to that in Vietnam. Cut down barrel and handle, with a webbing loop attached to the butt. He carried it looped around his neck with a buckshot or CS round.
He said it was a heavy son of a gun that way, and made his neck hurt. But it had proved useful several times when breaking contact.
He was also big on working with shotguns in jungles.

I saw a similar photo attributed to a M79 captured from the VC during Vietnam. It was supposedly the result of trying to salvage a workable weapon from a damaged weapon.
Probably not, the 40mm is a low pressure round.
