Yeouch! What happened to bring that on?
I think the most common causes are excessive scratching or hitting the ear on a hard surface during vigorous shaking. In this case I don't believe it was from scratching but other than that I have no idea.

For some reason the vet is reluctant to drain the hematoma. I think the swelling is excessive but they went to vet school and I didn't so I am trusting them.

They stitch the ear up to produce scarring to add shape to the ear lost from cartilage damage.

We had a lab, her name was Goldie, that had the same problem from shaking and scratching. Turned out it was from allergies, she was allergic to virtually everything.

Our vet lanced and drained because he said if we let it reabsorb the ear would shrivel.

After a couple of relatively expensive test batteries we ended up doing what the vet originally suggested, giving her a daily prednisone tablet knowing that we were taking a little bit of the tail end of her life. We had her nine comfortable (for her) years.

Note: this is not staged. We found him like this.
We have two GSD's, a boy and a girl. Reno, the boy, loves his squeaky animals, he'll bring them to me for tug of war and fetch after he lets me win a tug of war. After a while he'll gather them together like your dog has and that is when Rebel, the female, tries to steal them. She isn't interested in them at all until Reno gathers them in. They go after it play fighting, although someone just visiting might think they were trying to kill each other. When I've had enough of them I'll tell them to knock it off. Rebel gives a an eff you look, Reno gives me a reproachful look and generally goes to lay on 'his' air conditioning vent.
