I think it just means the story one group would have you believe to be the truth. I was confused on this too since it has only recently started being used as often as it is now (maybe I wasn't paying attention before and it's been used all along)

Watch MSNBC or CNN's coverage of the Orlando incident. Listen to how it is described by Obama administration officials and Hillary Clinton.
Then compare that to what facts we have about the situation.
I'll use Todd's excellent illustration of this. (Miss you, buddy!)
Todd worked in the violent crimes unit of the DC AUSA's office. He worked under one Eric Holder. (You may have heard of him)
Eric Holder told his office that Washington DC didn't have any gangs in it. There were, perhaps, a couple of "crews" in DC, but no gangs. So as a result, Washington DC did not have a violent gang problem on Eric Holder's watch.
See how simple that is?
