Actually, it could swing to cover either of two door openings - room to sink/shower area, or sink/shower to commode room.
After thinking about it for a while, I figured out that the sign was trying to tell you that the handle did not rotate to unlatch the door, but was only to be used to push or pull the door into spring-loaded latches in both doorways.
Still, I found instructions for operating a door to be amusing.
