Was it one of those internal mechanism failures releasing the hammer as Tamara once hypothesized re a pre series 80 1911 in condition 1?

No, combo of pinned grip safety, oversized thumb safety, trigger shoe, and body movement. Safety came off in the holster, trigger got pulled via movement.
