A few paper Form 4 questions

So I bought a Octane 45 HD from a friend back in the fall. Finally arrived at my FFL about a month ago, just getting the form 4 filled out now.

Questions :

1. It looks like my FFL forgot to write Silencerco's full address (Box 4a). Is this a problem? Should I fill this in with ink?

2. It also looks like my FFL used a digital signature on this paper form. Will this be a problem?

3. It also appears that his digital signature is on the first, but not the second, ATF copy of the form 4. Does he need to sign the 2nd copy as well?

Just want to make sure I have this done correctly before I ship it off. If there are any issues I can bounce it back to my FFL for correction.
