Nevah been done befo'!

Did anyone really expect otherwise?

Deputy Secretary of Defense Patrick Shanahan tried to put the best face on things, telling reporters, “We failed the audit, but we never expected to pass it.”
ISTR that they said that when it was ordered.

No kidding. They just can’t shut down the supply system to fix things.

Causes enough problems to "ground halt" a single airframe or do a safety standdown when somebody done fucked up, shutting the entire system down to audit it...

I imagine there was a point where it could have been done. Sometime in the 1800s.

I knew of a guy who was cashiered for TDY fraud. (Pro tip: Repeatedly submitting large receipts that were written on office supply store receipt books will, eventually, get noticed.)

The CSM of a unit I was assigned to was demoted to SGM and forcibly retired because he thought his position entitled him to more TDY expenses than the Army allowed. It was a case of one aw-shit outweighing a thousand atta-boys, too; he was a pretty good CSM.

Yes, I know that CSM is a duty position and one remains an E-9 as an SGM, but being fired from that job is a de facto demotion.
